'the fellow after sharp blow' is the wordplay.
'the fellow' becomes 'he' (pronoun for a man).
'after' means one lot of letters go next to another.
'sharp blow' becomes 'swat' (I've seen this in another clue).
'he' put after 'swat' is 'SWATHE'.
