(I've seen this before)

'confused by how one' is the wordplay.
'confused' becomes 'addle' (I've seen this before).
'by' says to put letters next to each other.
'how one' becomes 'd' (I can't justify this - if you can you should believe this answer much more).
'addle'+'d'='ADDLED'
