'turning over' says the letters should be written in reverse.
'is'+'il'+'i'+'bt'='isilibt'
'isilibt' back-to-front is 'TBILISI'.

(Other definitions for tbilisi that I've seen before include "Capital city of Georgia" , "Georgian capital" , "Georgian place" , "Georgian city" .)
