(I've seen this in another clue)

'an' becomes 'a'.
'infinitesimally' becomes 'n' (I can't justify this - if you can you should believe this answer much more).
'small' becomes 'atomic' (similar in meaning).
'a'+'n'+'atomic'='ANATOMIC'
