'holding' is an insertion indicator.
'drunk' becomes 'sot' (sot can mean a drunken person).
'up' says the letters should be written backwards (in down clue: letters go upwards).
'sot' backwards is 'tos'.
'male' placed around 'tos' is 'MALTOSE'.
