I can't tell whether this definition defines the answer.

'patients here?' is the wordplay.
I cannot really understand how this works, but
'patients here?' could be 'ward' (I've seen this before) and 'ward' is found in the answer.
The remaining letters 'ins' is a valid word which might be clued in a way I don't understand.
This explanation may well be incorrect...
