'by doctor' becomes 'mo' (medical officer. I am not sure about the 'by' bit.).
'on' is a charade indicator (letters next to each other).
'empty' suggests removing the centre.
'seat' with its middle taken out is 'st'.
'inner'+'mo'+'st'='INNERMOST'
