'gangrene' can be an answer for 'tissue?' (I have seen 'Necrotic tissue ' mean 'gangrene' so perhaps 'tissue' could also mean 'gangrene'). I'm not certain of the 'decay of' bit.

'sick gran kelly upset about' is the wordplay.
'sick' indicates anagramming the letters.
'kelly upset' becomes 'gene' (I am not sure about this - if you are sure you should believe this answer much more).
'about' indicates putting letters inside.
