'spoil sarah with a' is the wordplay.
'spoil' becomes 'mar' (synonyms).
'sarah' becomes 'Sal' (Sally is a common pet form of Sarah).
'with' says to put letters next to each other.
'mar'+'sal'+'a'='MARSALA'
