(I have seen 'Feeling of discomfort' mean 'malaise' so perhaps 'feeling' could also mean 'malaise')

'westbound setting sail with a sick' is the wordplay.
I cannot quite understand how this works, but
an anagram of 'sail' is 'lais' which is present in the answer.
'a' is present in the answer.
The remaining letters 'me' is a valid word which might be clued in a way I don't understand.
This may be the basis of the clue (or it may be nonsense).
