'first aid' can be an answer for 'help' (I've seen this before). I'm unsure of the 'after an accident' bit.

'douglas say needs serious' is the wordplay.
I cannot quite understand how this works, but
'douglas' could be 'fir' (douglas fir is an example) and 'fir' is found in the answer.
'serious' could be 'staid' (I've seen this before) and 'staid' is found in the answer.

This may be the basis of the clue (or it may be nonsense).
