I cannot quite understand how this works, but
an anagram of 'note' is 'ento' which is located in the answer.
'doctor' could be 'mb' (Bachelor of Medicine, Medicinae Baccalaureus) and 'mb' is found in the answer.
This accounts for all the letters.
